First Back Pain Physio Appointment: What To Expect And Timeline

Back pain is frustrating because it interrupts the basics, sleeping, sitting, lifting, walking and training. The first physiotherapy appointment is where confusion gives way to clarity. You get a working diagnosis, a plan you can follow and the first steps to calmer movement.

What your first appointment includes

A typical initial assessment lasts 45 to 60 minutes. It has four parts that flow into one another so you leave with a clear plan.


  • History and goals. We discuss your story, symptom behaviour, training or work demands, previous episodes, medications and what success looks like for you. Night pain, pins and needles, bladder or bowel changes and recent trauma are screened gently but thoroughly.

  • Functional assessment. You will do simple movements, sit to stand, walk, bend, squat, heel and toe raises, sometimes a loaded hinge with a light kettlebell. We check what eases and what aggravates, whether the leg feels different side to side and how your back tolerates sustained positions.

  • Working diagnosis and explanation. We explain the most likely pain mechanism in plain language, for example irritable facet joints, a flexion or extension sensitive pattern, muscular spasm or nerve root irritation. If red flags or atypical findings appear, we outline next steps and, if needed, imaging or urgent referral.

  • First treatment and plan. You get hands-on therapy where appropriate, soft tissue work, joint mobilisations and strategies to calm muscle spasm. You also leave with 2 to 4 exercises matched to your presentation plus pacing guidance for the next 7 days.


Follow ups usually last 30 to 45 minutes and progress the plan, adjust loads and build resilience.

Hands-on care meets targeted exercise

Manual therapy aims to create a window of easier movement. Techniques such as joint mobilisation and soft tissue release are always explained and only done with your consent. The goal is not quick clicks for the sake of it, but calmer movement you can then reinforce with the right exercises.


Exercise is where durable change happens. We choose movements that fit your pain pattern and your life, not a one size fits all routine.

The myth of “The Big 3” and how we choose exercises now

You might have heard of The Big 3 for lower back pain, McGill curl-up, side plank and bird dog. They can be useful for some people some of the time, but they are not magic, not mandatory and not automatically right for your back. Modern practice is personalised, matching exercise to your dominant triggers and tolerances.


Examples:


  • If flexion is sensitive, you may start with extensions in lying, short prone props and supported hip hinging.

  • If extension is the irritant, you may start with knee hugs, seated flexion and supported box squats within comfort.

  • If standing is fine but sitting bites, we prioritise posterior chain strength and movement snacks across your day.

  • If walking sets you off, we explore stride length, cadence and calf or hip capacity.


Below are two gentle starters many irritable backs tolerate, plus two progressions when symptoms settle. Stop if pain spikes above 3 out of 10 or lingers worse into the next day.


Starter movements:


  1. Abdominal brace with heel slides

  2. Purpose, calm engagement of deep abdominal support without holding your breath.

  3. How, lie on your back with knees bent, gently draw your lower tummy in as if tightening a belt one notch, breathe normally, slide one heel out along the floor, then back. Alternate sides.

  4. Dosage, 2 sets of 8 to 10 slow slides per side, daily.

  5. Supported hip hinge to box

  6. Purpose, restore confident bending using hips rather than back guarding.

  7. How, stand in front of a chair or box, hold the back of the chair lightly, push hips back, keep chest proud, tap the box with your hips, return to stand. Pain should stay tolerable.

  8. Dosage, 3 sets of 8, 4 to 5 days per week.


Progressions:


  1. Dead bug with band reach

  2. Add a light band pulled overhead to increase trunk demand while keeping the spine quiet. 3 sets of 6 to 8 per side, every other day.

  3. Kettlebell hinge

  4. Use a light kettlebell, 6 to 12 kg as tolerated. Slide hips back, keep ribs down, stand tall. Start with 3 sets of 6, slow lowering, two to three days per week. Build load slowly if symptoms remain stable.


Your plan might include different choices if, for example, your pain centralises with extension or if nerve symptoms dominate. That is the point, it is personalised.

Red flags, including night pain, and when to seek urgent care

Most back pain is mechanical and settles with a good plan. Some features need prompt medical assessment:


  • Night pain that is severe, progressive and does not change with position over several nights

  • Unexplained weight loss, fever or a history of cancer

  • Recent major trauma, especially with osteoporosis risk

  • New bladder or bowel dysfunction, saddle numbness or rapidly worsening leg weakness

  • Widespread numbness or tingling that is new or worsening


Night pain can be a red flag when it is relentless and unresponsive to position changes. Many people with mechanical back pain also notice some night discomfort, especially when rolling or after a big day, which is common and usually not sinister. If you are unsure, ask. How fast will you see change and how to know it is working

Change timelines vary with irritability, sleep, stress and training or work demands. Typical patterns we see:


  • Early change, within 1 to 3 sessions, easier movement after hands-on work plus exercises, reduced guarding and a clearer idea of what helps or hinders

  • Short term milestones, by 2 to 4 weeks, better tolerance for sitting or standing, smoother bending, shorter morning stiffness and control of flare ups

  • Medium term milestones, by 4 to 8 weeks, improved strength, capacity for longer walks, gym foundations and confidence with lifting


You will know therapy is working when pain is trending down week to week, you can do more before symptoms appear, flare ups settle faster and your home plan feels logical rather than random. If progress stalls, we re-test, adjust and, if appropriate, consider needling or imaging referral.

Is seeing a physio worth it and what if progress is slow

Physiotherapy is often worth it if you value rapid access, one-to-one time and a plan that blends hands-on care with progressive loading. If you have already tried rest and painkillers without lasting change, targeted rehab usually outperforms passive strategies alone.


When to reconsider or change tack:


  • No meaningful change after 3 to 4 sessions despite good adherence

  • Red flags emerge or neurological signs progress

  • Your goals or circumstances shift and you prefer self-management
